Client Informed Consent & Telehealth Agreement

By participating in the IBS Clarity Program and attending consultations with Connected Nutrition / IBS Dietitian Angele, I acknowledge and agree to the following:

Nature of Services

I understand that the services provided are delivered by an Accredited Practising Dietitian and consist of nutrition education, dietary assessment, nutrition counselling, and personalised nutrition recommendations.

I understand that these services are not intended to diagnose, treat, cure, or prevent medical conditions and do not replace medical advice provided by my doctor or other healthcare professionals.

Personal Responsibility

I acknowledge that I am responsible for my own health decisions and implementation of any recommendations provided.

I understand that outcomes vary between individuals and no guarantees regarding specific health outcomes, symptom improvements, or results can be made.

Medical Conditions

I agree to provide accurate and complete information regarding my medical history, symptoms, medications, supplements, allergies, and other relevant health information.

I understand that failure to provide accurate information may affect the suitability of recommendations provided.

Telehealth Services

I understand that consultations may be conducted via telehealth platforms such as Zoom.

I acknowledge that telehealth may involve limitations including:

• Internet or technology interruptions
• Reduced ability to perform physical assessments

I accept these limitations and consent to receiving services via telehealth.

Communication Between Consultations

I understand that communication outside scheduled consultations may be limited to administrative matters unless otherwise stated within the Program.

The service is not designed to provide emergency medical support.

Emergency Situations

I understand that Connected Nutrition does not provide crisis, emergency, or urgent medical services.

If I experience severe symptoms, medical emergencies, thoughts of self-harm, or urgent health concerns, I will seek immediate assistance from my GP, local hospital, or emergency services.

Privacy and Confidentiality

Recording of Group Sessions
